The endo-perio lesion is a pathology that involves the pulp and periodontal tissue due to their intimate relationship through the apical foramen and the accessory canals, which can transmit the pulpal infection to the pulp tissue and vice versa. The objective of this work was to present a case report and the endodontic and periodontal treatment of a tooth that was totally compromised by the endo-perio lesion and grade II mobility. Patient came complaining of pain and mobility in teeth 36 and 37. Radiographic examination showed that one of the teeth had an unsatisfactory endodontic treatment and the other had a great loss of bone structure and mobility grade II with probing depth of 7mm in the distal region. The chemical-mechanical preparation was performed using the EasyLogic system and accompanied by abundant 2.5% sodium hypochlorite intracanal medication for 15 days and scaling and root planing with McCall and Gracey curettes, after which there were no more symptoms. and both teeth were filled. Radiographic follow-up was performed after 2 years of this treatment, showing the evolution of lamina dura and periodontal tissue formation, with successful therapy.
