Abstract

Intraventricular hemorrhage (IVH) is one of the main neurological complications, especially in critically ill patients and premature neonates, and is associated with high morbidity and mortality and risk of permanent neurological sequelae. This study aimed to analyze the main strategies for managing and preventing intraventricular hemorrhage, based on the most recent scientific evidence. This is a systematic literature review, conducted in the PubMed, SciELO, LILACS and BVS databases, using descriptors related to intraventricular hemorrhage, treatment, and prevention, combined with Boolean operators. Primary studies published in the last five years, in English, Portuguese, and Spanish, addressing clinical and pharmacological interventions and preventive measures were included. The findings show that the management of IVH involves intensive support, rigorous control of intracranial pressure, ventricular drainage when indicated, and judicious use of adjuvant therapies. In the preventive context, measures such as hemodynamic control, use of antenatal corticosteroids, appropriate management of mechanical ventilation, and neuroprotective strategies stand out. It is concluded that an early and multidisciplinary approach, associated with the implementation of evidence-based preventive protocols, is fundamental to reducing the incidence and improving clinical outcomes related to intraventricular hemorrhage.
