Occupational accidents related to agricultural machinery and pesticides are relevant events for workers’ health and safety, particularly in territories characterized by agricultural and agro-industrial activities and the use of chemical inputs. This study aimed to describe the incidence of work-related accidents reported to the Brazilian Notifiable Diseases Information System, Severe Work-related Accidents component, in the state of Pará, Brazil, from 2020 to 2025, for ICD-10 codes V84, W30, X48 and Y18, aggregated by Health Regions. This is a descriptive ecological study based on official secondary data from DATASUS/SINAN/ACGR and population estimates from the IBGE Population Base/POPS. Absolute frequencies, incidence rates per 1,000,000 person-years, state percentage contribution, regional incidence ratio relative to the state, incidence in the rest of the state and incidence ratio relative to the rest of the state were calculated. During the period, 77 V84 cases, 171 W30 cases, 60 X48 cases and 13 Y18 cases were recorded. Although state-level incidences were low, marked regional heterogeneity was observed, especially in Xingu, Carajás and Metropolitana II, depending on the code analyzed. The combined interpretation of the indicators made it possible to identify Health Regions with proportionally higher occurrence relative to the state pattern and to the rest of Pará. This study provides a regionalized overview of specific occupational events still scarcely explored in the state and contributes to identifying priority territories for surveillance, investigation and prevention in occupational health and safety.
