This study analyzed the incidence of tuberculosis in the macro-regions and micro-regions of Minas Gerais between 2020 and 2024, using data from the Notifiable Diseases Information System (SINAN) in a quantitative epidemiological approach. A total of 23,129 cases were recorded in the state, concentrated mainly in the Central, Southeastern, Eastern, and Northern regions, with particular emphasis on the urban centers of Belo Horizonte, Juiz de Fora, Uberlândia, Governador Valadares, and Montes Claros. A predominance of cases was observed in adults aged 20 to 59 years and in males. The analysis revealed significant regional inequalities in laboratory confirmation, indicating structural weaknesses and possible underdiagnosis in vulnerable areas. The results highlight territorial heterogeneity, the persistence of transmission in socioeconomically fragile regions, and reinforce the need to strengthen diagnostic capacity, intensify active case finding, and direct control actions to territories and populations at higher risk.
