Introduction: The aging population poses new challenges to health systems, especially regarding the prevention of Sexually Transmitted Infections (STIs) in the elderly, a population often overlooked in public policies. The invisibility of sexuality in old age contributes to the vulnerability of this group. Objective: To investigate the prevalence and factors associated with STIs in the elderly, proposing prevention and awareness strategies. Methodology: This is an integrative literature review, following PRISMA recommendations. The search was conducted in the BDENF, SciELO, and LILACS databases, covering publications between January 2019 and September 2025, resulting in the analysis of 14 studies. Results: The analyzed studies reveal a significant prevalence of STIs among the elderly, associated with a low perception of risk, inconsistent condom use, and a notable gap between knowledge about infections and the adoption of preventive practices. The lack of targeted health campaigns and the taboo surrounding sexuality in old age were identified as factors that exacerbate vulnerability. Conclusion: It is essential to deconstruct taboos about sexuality in old age, train health professionals for an appropriate approach, and develop specific public policies that integrate sexual health as an essential component of active and healthy aging.
