Brazil is facing an increasing burden of Chronic Non-Communicable Diseases, with an aging population posing challenges to the Unified Health System (SUS). This study aims to analyze SUS responses to the prevalence of these diseases, particularly among the elderly, and the challenges faced in ensuring continuity of care. The methodology adopted was a narrative literature review, selecting primary studies, systematic reviews, and public policy reports. The research was conducted in academic databases such as PubMed and SciELO, considering publications from 2020 to 2024. Results reveal that DNCTs are the leading public health issue in Brazil, with significant impacts on SUS costs. Continuity of care is perceived as fragmented, with difficulties in accessing specialized care and ineffective communication among professionals. Primary Health Care (APS), especially through the Family Health Strategy (ESF), is central to addressing these diseases. However, low integration between services in the Health Care Networks (RAS) and social inequalities hinder the effectiveness of care. The study concludes that improving the coordination of RAS and strengthening PHC are essential to ensure continuous and more effective care. Emphasizing self-care, investing in information technologies, and implementing integrated public policies are key to advancing the fight against DNCTs in Brazil.
