EVOLUTION OF MYELOFIBROSIS AND ACUTE MYELOID LEUKEMIA AFTER A CLINICAL PICTURE COMPATIBLE WITH SEVERE DENGUE: CASE REPORT

We report the case of a 44-year-old female patient with metabolic comorbidities who presented a clinical picture compatible with severe dengue, without laboratory confirmation, and later developed pancytopenia, deep vein thrombosis, pulmonary embolism, and disseminated fungal infection. Bone marrow biopsy revealed hypercellularity with diffuse myeloid proliferation and increased angiogenesis, consistent with proliferative myelofibrosis. Days later, peripheral blood smear showed 87% blasts, characterizing acute leukemic transformation. This case raises the hypothesis that a systemic viral infection similar to dengue may act as an immunoinflammatory trigger for severe hematopoietic disorders such as myelofibrosis and acute myeloid leukemia.
