This article sought to analyze, through an integrative literature review, the scientific evidence on the influence of diabetes on post-extraction healing and the necessary perioperative care. The methodology involved a search in the LILACS, PubMed, and SciELO databases (2020-2025), using the descriptors "Diabetes Mellitus," "Tooth Extraction," and "Postoperative Care," with the selection of 8 articles following PRISMA guidelines. The main results indicate that hyperglycemia is the central factor for delayed healing, due to inflammatory dysregulation and cellular dysfunction. Although glycemic safety parameters (< 200 mg/dL) are suggested, standardized protocols are lacking. Local adjuvant therapies, such as hyaluronic acid, have shown promise. It is concluded that the safe management of diabetic patients in tooth extractions requires a multifaceted approach, integrating rigorous glycemic control, meticulous surgical planning, and the use of local therapies to ensure predictable healing and minimize risks.
