The study aims to compare the mortality rate of gastrointestinal surgeries and associated organs in the state of Paraná with the national average, identifying factors that influence these rates and proposing improvements for surgical care. It is a cross-sectional epidemiological study, utilizing data from the Notification of Health Conditions Information System (SINAN) and Datasus. Paraná reported a mortality rate of 2.057%, slightly higher than the national average of 2.014%. Despite performing 7.043% of Brazil’s surgeries, the state’s mortality rate indicates potential issues with the health conditions of the population and hospital infrastructure. During the COVID-19 pandemic, Paraná recorded 118,434 digestive system surgeries, with a surgical mortality rate of 2.489%, higher than the national rate of 2.014%. The results suggest an urgent need for improvements in public health policies, surgical risk management, and training of healthcare professionals. Implementing adequate postoperative support and antibiotic prophylaxis is essential to reduce mortality and enhance the quality of life for surgical patients.
