Tuberculosis remains the most prevalent infectious disease worldwide. Brazil, a country of continental proportions, has seen an increase in the incidence of this disease in recent years. Objective: To analyze the incidence of tuberculosis cases between 2004 and 2023 in the Midwest region of Brazil. Methodology: An ecological study utilized four databases and compiled data to analyze the incidence and complications of tuberculosis cases in the Midwest over the period. Results: In the past two decades, the Midwest region reported the lowest number of tuberculosis cases compared to other regions. Data on hospitalizations and procedures related to tuberculosis showed a significant increase between 2007 and 2024, rising from 35 to 1.030. In contrast to the increase in procedures, vaccination coverage remained stable over these two decades, considering changes in administration methods. Conclusion: Despite significant progress in treatment and vaccination, and a lower number of cases compared to other regions, the Midwest still needs to take measures to optimize detection and treatment, as well as find ways to reduce the number of new tuberculosis cases.
