Heart failure with preserved ejection fraction is often associated with hypertension, which contributes to diastolic dysfunction and increased ventricular stiffness. Early identification of this dysfunction is crucial to prevent progression to heart failure. This study reviews the literature on the relationship between hypertension and diastolic dysfunction, emphasizing the importance of early intervention to prevent complications. Strict blood pressure control, the use of medications such as angiotensin-converting enzyme inhibitors and calcium channel blockers, along with the adoption of healthy habits like physical exercise and a balanced diet, are effective strategies. Personalizing treatment, considering the comorbidities of each patient, is also essential. Despite the demonstrated benefits, adherence to treatment and determining the ideal timing for intervention remain challenges. Continued research is needed to improve management and the quality of life of patients.
