Chronic hip pain in young patients can have diverse and challenging etiologies. This report describes the case of a 25-year-old patient treated at a municipal hospital in southern Bahia, presenting with insidious pain in the right hip without a history of trauma. After months of investigation, laboratory tests revealed a slight elevation in inflammatory markers and a positive FABER test. The final diagnosis was persistent transient synovitis associated with myofascial dysfunction. Conservative treatment with functional rehabilitation and analgesia led to symptom resolution. This case highlights the importance of detailed clinical evaluation and differential diagnosis in orthopedics, especially when imaging is not an initial diagnostic option.
