Abnormal uterine bleeding (AUB) is a common gynecological condition that affects women in different age groups, resulting in a significant impact on quality of life. The clinical management of SUA may involve several approaches, with the use of levonorgestrel intrauterine devices (IUDs) being an effective and widely adopted therapeutic option. This article aims to review the use of the levonorgestrel IUD as an alternative in the treatment of abnormal uterine bleeding, discussing its mechanisms of action, efficacy and safety. A literature review was carried out through the main medical databases using the descriptors “intrauterine device”, “levonorgestrel” and “abnormal uterine bleeding”, using the Boolean operator “AND”. All articles published between 2019-2024 were included in the primary analysis Clinical studies demonstrate that the use of the LNG IUD significantly reduces the volume and duration of menstrual bleeding in women with AUB, in addition to relieving pelvic pain associated with it. In addition to its effectiveness in controlling bleeding, the levonorgestrel IUD also has advantages over other therapeutic options, such as oral contraception, due to its local action and lower occurrence of systemic side effects. Therefore, the levonorgestrel IUD is a highly effective and well-tolerated therapeutic option in the treatment of abnormal uterine bleeding.
