This study analyzed HIV/AIDS mortality in Paraná between 2014 and 2023, identifying a 34% drop in deaths, but revealing persistent inequalities by sex, age group, race/color, and region. Men and adults between 30 and 49 years old were the most affected, and the concentration of deaths in the Eastern Macroregion, especially in the Metropolitan Region of Curitiba, indicates inequality in the distribution of health services. The COVID-19 pandemic impacted access to diagnosis and treatment, leading to a temporary drop in deaths in 2020 and 2021, followed by an increase in 2022, with the resumption of services. The research highlights the importance of decentralizing services, using PrEP, and combating stigma to improve treatment adherence and expand prevention. To effectively address the epidemic, it is necessary to maintain continuous monitoring and adapt public policies to the specific needs of vulnerable populations and underserved regions, thus promoting more equitable and inclusive care.
