Endodontics deals with the health of the dental pulp and periapical tissues, seeking to disinfect and shape the root canals. When endodontic treatment does not solve the problem, endodontic surgery emerges as an alternative, being planned to remove persistent infections. Conduct a bibliographic review on the ineffectiveness of endodontic treatment and the use of endodontic surgery as an alternative intervention. This is a literature review, with articles published in the last sixteen years, in Portuguese, English and Spanish, using the Health Science Descriptors, with a final sample of 19 articles. The results analyzed indicate that endodontic surgery is an effective alternative, being widely indicated in cases of endodontic failure, as long as it is well planned and associated with appropriate techniques, in addition to biocompatible materials. There are several techniques that include apicoectomy, curettage and removal of fractured instruments, with high success rates when performed by trained professionals. It is concluded that parethodontic surgery is an excellent choice in cases where periapical lesions persist after conventional endodontic treatment and retreatment, which makes it clear that it must always be accompanied by a good diagnosis and adequate treatment plan, and the postoperative period is crucial.
