SURGICAL AND DERMATOLOGICAL APPROACH TO THE TREATMENT OF CHRONIC VENOUS ULCERS ASSOCIATED WITH DEEP VENOUS INSUFFICIENCY

Introduction: Chronic venous ulcers, often associated with deep venous insufficiency, represent a significant challenge in clinical practice, affecting the quality of life of patients, especially women. These ulcers result from a combination of factors, including hyperinflammation, impaired healing, and alterations in microcirculation. Treatment involves surgical and dermatological approaches, aiming not only at healing the lesions but also at preventing recurrences. The complexity of the condition requires a multifaceted strategy, including the management of risk factors, such as obesity and family history, which are particularly relevant in women. Objective: To explore surgical and dermatological approaches in the treatment of chronic venous ulcers associated with deep venous insufficiency, highlighting their efficacy and clinical implications. Methodology: The methodology adopted the PRISMA checklist to ensure rigor in the selection of studies. The databases consulted were PubMed, Scielo, and Web of Science, using the descriptors Topical medications, Cell growth, Angiogenesis, Growth factors, and Quality of life. The inclusion criteria were: studies that addressed the relationship between surgical and dermatological treatment in venous ulcers, publications from the last 10 years and that included representative samples of women. Articles that did not directly address venous ulcers, literature reviews and studies in animal models were excluded. Results: The results demonstrated that the combination of surgical interventions, such as the removal of insufficient veins, and dermatological care, including the use of advanced dressings, significantly improved healing rates. It was observed that the integrated approach not only promoted ulcer healing but also reduced the incidence of recurrences, especially in women, who benefited from a management focused on their specific needs. Conclusion: The surgical and dermatological approach in the treatment of chronic venous ulcers associated with deep venous insufficiency proved to be effective, highlighting the importance of integrated strategies to promote healing and quality of life of patients. The literature highlighted the relevance of considering the specific characteristics of women, to ensure optimized results in the management of this challenging condition.
