Introduction: Society often stigmatizes drug addicts with prejudice and discrimination, which creates substantial barriers to seeking treatment. Fear of being judged or marginalized can cause these individuals to isolate themselves and avoid support essential to their recovery. Objective: aims to identify factors that hinder adherence to treatment, including aspects related to social stigma, psychological and social factors, and the effectiveness of available treatment programs. Methodology: This is an integrative review study, with a qualitative approach using a search for scientific productions in the Scientific Electronic Library Online (Scielo) and Virtual Health Library (VHL) databases, from 2019 to 2023. Results and Discussion: It is observed that relapse prevention and maintenance of abstinence require continuous support and effective coping strategies and the absence of a solid social support network can be a major obstacle for drug addicts during treatment and recovery. Final considerations: It should be noted that social stigma, limited access to resources, comorbidities, relapse and lack of social support are just some of the difficulties faced by those who struggle with addiction. However, with integrated approaches, adequate support and empathetic understanding, it is possible to overcome these challenges and promote a successful and lasting recovery for drug addicts.
