This article sought to demonstrate that placing dental implants immediately after extraction has significant advantages over delayed techniques, such as reducing the number of surgeries and treatment time. Comparative studies between immediate and delayed techniques have shown greater success in preserving tissue and bone architecture with the immediate technique, especially when using a customized provisional restoration. However, in the molar region, immediate implantation presents challenges due to anatomical limitations, requiring larger implants. Despite its advantages, it should be applied in specific cases, taking into account the patient's biotype and accurate assessment of peri-implant tissues. 3D scanning technologies are recommended for better evaluation. Patient acceptance is greater with the immediate technique, especially in anterior teeth, where esthetics are essential. Studies also highlight the importance of grafts in maintaining bone architecture after immediate implantation, although the results between the groups with and without grafts did not show significant differences in the long term. Even with care, a small vestibular bone resorption can occur, and several techniques, such as the use of grafts and preservation of soft tissues, are suggested to mitigate this effect.
