Objective: To investigate the risks and unfavorable obstetric outcomes in women with endometriosis. Method: We searched the Medline database for eligible studies from 2010 to 2020, without restriction. After filtering free complete articles, two researchers read the abstracts and titles of the studies and subsequently cross-checked the data and resolved the discrepancies together with a third researcher. 21 studies were selected for full reading, where the inclusion criteria were applied - studies from cohort type that compared obstetric outcomes between women diagnosed with endometriosis and those without endometriosis - articles that compared exclusive outcomes of women who underwent medically assisted reproduction methods were excluded. 7 studies met the criteria for this review. To guarantee the quality of the methodology, the PRISMA criteria were met at all stages of the development of this systematic review. Results: The main outcomes found in the systematic review were the incidence of spontaneous abortion, placenta previa, placental abruption and premature birth. The articles reviewed corroborate the hypothesis that endometriosis can be considered a risk factor for both the mother and the newborn. Conclusion: Endometriosis can interfere with the physiological course of pregnancy, this study showed that the disease can lead to an increased incidence of spontaneous abortion, an increasing development of premature birth, and also carries significant risks of various complications, such as pre-eclampsia and placental complications in pregnancy and childbirth. Therefore, it is recommended that health professionals carry out adequate monitoring of pregnant women already diagnosed with endometriosis.
