Doxazosin, an alpha-1-adrenergic blocker, is widely used in the treatment of arterial hypertension and benign prostatic hyperplasia (BPH). However, postural hypotension (PH) is a common side effect associated with its use, which can significantly impact patients' quality of life and treatment adherence. This study aims to evaluate the incidence of postural hypotension in patients treated with doxazosin and discuss the risk factors and strategies to minimize this adverse effect. This is a review of clinical studies and meta-analyses that investigated the incidence of postural hypotension in patients using doxazosin, including data from different populations and dosages. It was observed that doxazosin has a considerable incidence of postural hypotension, especially in the initial phases of treatment or after dose increases. Elderly patients and those with cardiovascular comorbidities are more susceptible. Measures such as gradual dose adjustment and nocturnal administration of the medication may help reduce the incidence of PH. Postural hypotension is a relevant side effect of doxazosin treatment, requiring monitoring and preventive strategies to ensure the safety and efficacy of the treatment.
