Uterine fibroids, benign tumors that impact patients' quality of life, have been the target of considerable advances in the understanding of their pathophysiology and available surgical options. The individualized approach, considering size, location and symptoms, is highlighted, with an emphasis on less invasive alternatives, such as robotic surgery and uterine embolization. The objective was to list the best surgical technique for managing the condition, as well as to establish the variables associated with complications and complications in each scenario. Among the determinants of the disease, factors such as age, ethnicity, parity, hormonal aspects, body mass index and coexisting medical conditions stand out. Therapeutic options, including laparoscopic myomectomy, hysteroscopic myomectomy, and uterine embolization, are discussed, highlighting benefits, risks, and specific considerations for each technique. The need for treatments that preserve women's fertility is concluded, and a critical view can be inferred about the lack of consensus in the literature regarding the superiority between surgical approaches, emphasizing the importance of additional studies.
