Traumatic injuries, particularly those involving the chest, represent a significant public health issue due to their high morbidity and mortality rates. Among these, penetrating cardiac injuries are especially severe, with the Ziedler zone—a specific anatomical region of the thorax—being a critical area due to its potential involvement with the heart. This case report examines the management of a 33-year-old male patient who sustained a stab wound to the precardial area, identified as being within the Ziedler zone. The initial approach included the implementation of the Advanced Trauma Life Support (ATLS) protocol, which was crucial for the patient’s assessment and management. The ATLS protocol provides a structured framework for the treatment of trauma victims, aiding in the identification and treatment of life-threatening injuries. In this case, the patient exhibited signs of hypovolemic shock and cardiac tamponade, necessitating urgent exploratory thoracotomy. Surgical intervention revealed pericardial effusion and a hematoma, which were successfully addressed. The prompt hemodynamic stabilization and appropriate therapeutic measures resulted in favorable clinical outcomes. This case highlights the importance of early diagnosis and immediate treatment in thoracic trauma, particularly in critical areas like the Ziedler zone, to improve survival rates and minimize complications.
