There is an estimated increase in the number of elderly populations around the world, including in Brazil, which raises concerns regarding the maintenance and improvements in public policies aimed at this population. In Piauí, this process of increasing the number of elderly people will also occur in addition to the increased inclusion of elderly people as low-income people in the State, which has an impact on the quality of life and health of elderly people in the State. This could lead to an increase in the number of illnesses and hospitalizations, as well as the filing of individual demands. In this scenario, this article seeks to discuss the contributions offered by the structural process for the implementation of public health policies for elderly people in Piauí. The research is qualitative in approach, of a bibliographic and documentary nature, through the analysis of Jurisprudence of the Court of Justice of the State of Piauí and State health reports. Advantages of the structural process for implementing improvements in the health of elderly people were identified as a result, especially due to the polytricentric nature of the litigation and because it is a litigation of a structural nature.
