Cesarean section is a way of delivering babies surgically under medical advice. The growing incidence of cesarean sections in Brazil has been a cause for concern, both nationally and regionally, especially with regard to maternal and neonatal health. Although it is necessary to respect a woman's right to choose the mode of birth she deems most appropriate, given the many factors that can influence this decision. Objective: to investigate the profile of births carried out in the city of Ilhéus-Bahia in the year 2022 and identify which factors influence the choice of this type of birth according to data available in DATASUS. Materials and Methods: this is an epidemiological study, of an ecological type. The data collected were on live births (SINAN) from cesarean sections in the year 2022. Sociodemographic variables, obstetric clinical conditions and characteristics of the newborn (NB) were analyzed. Results and Discussions: A total number of live births in Ilhéus of 2030 were recorded in the year 2022, of which 45.9% (931) were cesarean sections. The relevance of studying the profile of cesarean births in Ilhéus, Bahia, is extremely important due to a series of factors that affect both public health and medical practice in the cocoa region. Conclusion: The discussion sought to highlight the importance of the results found to guide health policies and clinical practices aimed at reducing unnecessary cesarean section rates, thus promoting maternal and neonatal health and guaranteeing the right of pregnant women to safe, evidence-based obstetric care.
