Patients with subarachnoid hemorrhage may have cardiovascular manifestations, and it is important to be aware of the different differential diagnoses. Takotsubo cardiomyopathy is part of these diagnoses, being a pathology that both clinically and electrocardiographically can mimic an acute coronary syndrome. When associated with alteration of consciousness, it’s diferencial diagnosis is increasingly difficult. Two clinical cases of female patients are described who presented clinically and electrocardiographically with signs of acute coronary syndrome, but who were subsequently diagnosed with Takotsubo cardiomyopathy. Both cases relate to an atypical presentation of SAH that should raise suspicion for the correct diagnosis and treatment to the critical care patient medical team.
