Introduction: The Radicular Cyst (RC) is classified as an inflammatory apical odontogenic cyst and its cause is associated with the epithelium of the apex of a dental unit with pulp necrosis, developed by an inflammatory process. This lesion does not present painful symptoms and grows slowly, reaching large proportions, presenting characteristics such as mobility, swelling and sensitivity. Objective: The objective of this work was to report a clinical case of periapical cyst, demonstrating the diagnosis and proposed surgical therapy. Case report: A 23-year-old female patient, Caucasian, attended the oral and maxillofacial surgery and traumatology outpatient clinic at the Oswaldo Cruz University Hospital, reporting an increase in volume in the anterior region of the maxilla. On intraoral examination, there is an increase in volume and crepitus in the right anterior region of the maxilla. The radiographic examination showed a radiolucent, circumscribed and well-defined area between the roots of the right central, lateral and canine incisors, suggestive of a periapical cyst. The proposed treatment was enucleation and curettage of the lesion under local anesthesia. The collected material was sent for anatomopathological analysis. The patient returned after 7 days, without signs of infection, debt or suture dehiscence. Conclusion: The present work addressed the surgical treatment of a radicular cyst, therefore, it is important to include complementary diagnostic tests, such as radiographic and histopathological examinations, in the dental routine. The success of this procedure depends on correct case planning, in addition to knowledge of head and neck anatomy and surgical techniques.
