REPORT OF CLOSURE OF BUCCAL-SINUS FISTULA WITH BUCCAL FAT PAD AND VESTIBULAR FLAP

Oral-sinus or oroantral communication is a complication that can occur as a result of surgical trauma, more common in extractions where the tooth has an intimate relationship with the maxillary sinuses. This study aims to describe in detail a clinical case of oral-sinus communication in a 66-year-old man who sought the service complaining of pain and bad smell in the mouth. The first intervention, which used only the vestibular flap, was unsuccessful, resulting in an episode of recurrence. The second intervention used the vestibular flap associated with the cheek fat pad and showed no signs of recurrence. These conditions must be treated immediately due to the risk of developing into chronic sinusitis conditions. The main techniques, whether through transposition of the vestibular flap or the use of bichat fat, are easy to obtain and manipulate, do not require specialized material and can therefore be performed in a dental outpatient clinic.
