The anatomical diversity of the bile ducts is widely documented, generally not representing a significant risk to patients. However, clinical and surgical complications may arise in certain contexts, such as during cholecystectomy procedures, liver transplantation, hepato-biliary approaches for tumor resection or drainage. This study presents the case of a female patient who presented to the emergency room with pain in the right upper quadrant of the abdomen, accompanied by fever and changes in the results of laboratory tests. After performing an MRI cholangiography, an anatomical variation in the extrahepatic bile ducts was identified. A complete understanding of this anatomical variability is crucial for general surgeons, especially considering that cholecystectomy is a common procedure and the leading cause of iatrogenic bile duct injuries.
