The article addresses the evolution of Clinical Pharmacy and the transforming role of the pharmacist in health management. Over time, we observe a transition from the traditional role of dispensing medications to a central figure in integrated health management, emphasizing the importance of a patient-centered approach. The integrative systematic review reveals the positive impact of multidisciplinary collaboration in patient management, emphasizing the customization of care as cr9ucial for therapeutic efficacy and overall well-being. In the context of pharmaceutical care in Montes Claros, Brazil, the research highlights its critical importance, emphasizing challenges such as inventory management and efficient integration of pharmaceutical professionals. Graphs provide insights into the increasing integration of pharmacists into healthcare teams and the assessment of pharmaceutical care in basic health units. Despite notable advancements, challenges such as the need for continuous education and enhanced communication skills are recognized to address the expanding clinical role of the pharmacist. In summary, the research contributes to a comprehensive understanding of the evolution of Clinical Pharmacy and the pharmacist's role in health management, providing a solid foundation for improvements in the integration of pharmacists into contemporary healthcare, with a focus on a more effective and patient-centered approach.
