Abstract

Third molars are the last permanent teeth to erupt and have a high prevalence of impaction, which favors the development of odontogenic cysts, especially dentigerous cysts. This lesion originates from the accumulation of fluid between the reduced enamel epithelium and the crown of an unerupted tooth. In most cases, dentigerous cysts are asymptomatic and identified incidentally on routine radiographic examinations. A definitive diagnosis is established through histopathological examination. To gather and analyze recent scientific evidence on dentigerous cysts associated with impacted third molars, emphasizing clinical, radiographic, diagnostic, and therapeutic aspects, in order to support surgical planning and clinical management. A literature review was conducted covering publications between 2015 and 2025. Studies, such as case reports, related to dentigerous cysts in impacted third molars were included. The analysis demonstrates that dentigerous cysts are the most common developmental odontogenic lesion, commonly associated with mandibular third molars. The most commonly used therapeutic options are enucleation combined with tooth extraction and marsupialization for larger lesions. Early recognition of dentigerous cysts and appropriate management of impacted third molars are essential to prevent complications and preserve anatomical structures. An integrated analysis of diagnostic and therapeutic aspects contributes to safe, evidence-based clinical management.
