Abstract

Introduction: The search for minimally invasive aesthetic procedures for facial harmonization has grown exponentially. Rhinomodeling with botulinum toxin type A (Botox) emerges as an alternative to surgical rhinoplasty, with shorter recovery time and reduced risks. However, a direct comparison between the two approaches regarding aesthetic efficacy, durability of results, and safety still lacks a robust systematic review. Objective: To compare the efficacy and safety of rhinomodeling with botulinum toxin type A with surgical rhinoplasty. Methods: Systematic review following the PRISMA guidelines, based on clinical trials, observational studies, and case series published between 2013 and 2024 in the PubMed, Embase, Scopus, and LILACS databases. Inclusion criteria: Adults undergoing rhinoplasty or Botox rhinomodeling, with assessment of aesthetic and safety outcomes. Six studies were selected after screening 194 references. Results: Rhinoplasty demonstrated more lasting results and more comprehensive structural corrections, with an average recovery time of 15 to 30 days. Rhinomodeling showed good immediate aesthetic acceptance, rapid recovery (<48 hours), and a high safety profile, but with a temporary effect (3–6 months). Adverse events were rare in both groups, but the studies highlighted the need for greater standardization in the evaluation of rhinomodeling results. Conclusion: Both approaches are aesthetically effective, but with different profiles. Rhinoplasty offers definitive results, with greater surgical complexity, while Botox rhinomodeling is safe, rapid, and indicated for mild to moderate corrections. The choice should consider the patient's goals, nasal anatomy, and individual risk-benefit.
