Squamous cell carcinoma (SCC) is the second most common type of non-melanoma skin cancer, with an estimated prevalence of 20% of cutaneous carcinomas. This report addresses the management of a giant SCC in the forehead of an elderly patient, with emphasis on the surgical approach by flap rotation and cranial trepanation. Methods: Case report. Medical records and photographs were reviewed, and a comprehensive literature review on the subject was performed. Results: The patient underwent oncological resection of a tumor lesion in the forehead, followed by flap rotation based on the superficial temporal artery, achieving satisfactory results from the aesthetic, functional and oncological control points of view. Conclusion: The present study emphasizes early diagnosis and a multidisciplinary approach to squamous cell carcinomas, highlighting in the context of the Legal Amazon the need for public policies, screening and education to reduce advanced cases and morbidity and mortality.
