This case report presents an open orchiectomy performed on a ram with a right testicular abnormality. A male, mixed-breed sheep was presented with a significant enlargement of the right testicle, initially suspected to be a traumatic injury. After the condition worsened and further exams were conducted, a testicular infection with purulent fluid was diagnosed. Initial treatment involved the administration of anti-inflammatory drugs and antibiotics, but due to unsatisfactory results, bilateral open orchiectomy was chosen. The open orchiectomy technique was selected due to the severity of the infection, following veterinary surgical protocols. The surgery was performed under sedation and local anesthesia, with both testicles removed, and the right testicle drained due to purulent fluid. Postoperative care included blood transfusions, antibiotic and anti-inflammatory treatments, as well as rigorous wound management. Postoperative complications included hemorrhage and edema. Blood transfusions and compressive dressings were required to manage these complications. The ram was discharged after a full clinical recovery. Thus, the surgical and therapeutic management proved effective, though challenging due to the postoperative complications.
