Hormone therapy (HT) has been widely used to mitigate symptoms associated with menopause, significantly impacting the quality of life of postmenopausal women. This longitudinal review assessed the effects of HT over time, considering a wide range of studies that included clinical trials, cohort studies and systematic reviews. The results indicate that HT offers substantial benefits in reducing vasomotor symptoms, improving mental health and sexual function in postmenopausal women. However, these benefits tend to diminish over time, while risks, such as increased incidence of breast cancer and cardiovascular events, become more pronounced with prolonged use. The review highlights the need for an individualized approach and continuous reassessment of the risks and benefits of HT, ensuring that patients' quality of life is optimized. This study reinforces the importance of informed and patient-centered clinical decisions, based on a balanced analysis of the benefits and potential adverse effects of HT.
