THERAPEUTIC APPROACHES FOR THE MANAGEMENT OF RESISTANT HYPERTENSION: AN INTEGRATIVE REVIEW

Abstract

Resistant hypertension (RHTN) is a challenging clinical condition characterized by persistently high blood pressure levels despite treatment with three or more antihypertensive medications. This integrative review aims to analyze current and emerging therapeutic approaches in the management of RHTN, focusing on pharmacological, non-pharmacological therapies, and technological innovations. The review included 32 relevant studies, addressing the efficacy of mineralocorticoid receptor antagonists (MRAs), renal modulation therapies, lifestyle changes, and new technologies such as baroreflex stimulation. The results show that the combination of MRAs with other antihypertensive agents is effective, but monitoring for adverse effects is crucial. Non-pharmacological interventions, such as weight loss and reduction of sodium intake, also demonstrate significant benefits. Emerging innovations, such as baroreflex stimulation devices, offer new perspectives but require further studies for extensive validation. It is concluded that a multifaceted approach, integrating pharmacological therapies, lifestyle changes and technological innovations, is essential to optimize the management of resistant hypertension.
