This article aimed to evaluate the efficacy of surgical treatment in complicated acute appendicitis in emergency care. The methodology employed followed the PRISMA methodology, with research in the PubMed, Cochrane, SciELO, and BVS databases, covering the last ten years (2013-2023). Clinical studies, systematic reviews, and meta-analyses that evaluated the efficacy of appendectomy in cases of complicated acute appendicitis were included. The main results found indicate that early appendectomy significantly reduces postoperative complications and improves recovery times. Studies highlight the importance of rapid surgical intervention to minimize the risk of perforation and other complications. It is concluded that appendectomy remains the gold standard treatment for complicated acute appendicitis, with robust evidence of its efficacy in reducing complications and faster recovery times. Early surgical decision-making and the availability of adequate resources are critical factors for successful treatment.
