The objective of this work, then, is to report the clinical case, with surgical management of an aggressive condition of psammomatoid juvenile ossifying fibroma, with involvement of the orbital region, sphenoid and ethmoidal sinuses, maxillary sinus and extension to the maxilla. MEMR patient, 12 years old, female, with a history of increased volume in the right hemiface. In September of the same year, a biopsy was performed by an otorhinolaryngologist due to sinusitis, resulting in the diagnosis of psammomatous ossifying fibroma. FOPJ frequently occurs in the paranasal sinuses, mainly in the frontal and ethmoid sinuses, but in the case described, the lesion affected the entire right maxilla, infraorbital region together with the orbital floor, nasal fossa, right maxillary sinus, palate and dentoalveolar region. The surgical proposal consisted of surgical excision with wide curettage of the lesion, under general anesthesia. The patient had a good postoperative course, with no tissue dehiscence and no inflammatory or infectious conditions. With this, we conclude that the accurate diagnosis must be made through definitive biopsy and a correlation must be established based on clinical, radiographic and histopathological findings for an accurate diagnosis.
