In the last century, Alberto Urrets-Zavalia Jr. published an article based on Castroviejo's description of the fixed dilated pupil. This condition was later named Urrets-Zavalia syndrome in honor of the researcher. This syndrome can be seen in patients who undergo ophthalmological surgeries. However, there is a lot of speculation and little clarity about its risk factors, its pathophysiology and its etiology. Therefore, its study, detailing, description and updating must be carried out more frequently, so that a better understanding of the clinical picture of the disease is established. Therefore, this narrative literature review brought together articles from the main databases with the aim of pointing out the main correlations of Urrets-Zavalia syndrome with other diseases and the performance of ophthalmological surgical procedures, in addition to describing its main clinical characteristics. It was concluded that the syndrome appears postoperatively, especially in patients diagnosed with keratoconus who undergo corneal transplantation. In addition to keratoplasty, the correlation with secondary glaucoma, the postoperative period of cataract surgery and intraocular lens implantation, trabeculotomy, goniotomy and peripheral iridoplasty with argon laser should also be highlighted. Another important correlation is the diseases that affect the corneal tissue.
