Rehabilitation of the posterior maxilla with dental implants represents a significant clinical challenge due to alveolar bone resorption and maxillary sinus pneumatization, which reduce bone availability. In this context, maxillary sinus floor elevation is an important surgical approach to enable implant placement. This study aimed to analyze the effectiveness of leukocyte- and platelet-rich fibrin (L-PRF) as an adjunct in this procedure, focusing on bone regeneration, tissue healing, and clinical outcomes. A descriptive and qualitative literature review was conducted using PubMed/MEDLINE, BVS, Embase, SciELO, Elsevier, and Google Scholar, covering publications from 2021 to 2026. The findings indicate that L-PRF exhibits biological properties associated with angiogenesis, cell proliferation, and the sustained release of growth factors, including PDGF, VEGF, and TGF-β. Its application may promote tissue healing, reduce postoperative complications, and contribute to bone regeneration, particularly when combined with bone grafting biomaterials. However, heterogeneity in clinical protocols and available evidence limits definitive conclusions regarding its superiority. In conclusion, L-PRF represents a promising adjunctive biomaterial in maxillary sinus floor elevation procedures, although further clinical studies are required to confirm its benefits and standardize its application in dental practice.
