Contemporary dentistry constantly seeks strategies and biomaterials capable of optimizing bone regeneration and tissue repair, with particular emphasis on autologous biomaterials due to their high biocompatibility. In this context, Leukocyte- and Platelet-Rich Fibrin (L-PRF) has emerged as a second-generation platelet concentrate obtained from whole blood through a simplified centrifugation process without the use of chemical additives or anticoagulants. The resulting three-dimensional fibrin network enables the sustained release of growth factors and cytokines, which play essential roles in neoangiogenesis, inflammatory modulation, and the transition from passive healing to guided regeneration, helping to overcome vascular, structural, immunological, and cellular limitations. However, the lack of standardization in centrifugation parameters and the limited methodological robustness of some current clinical studies still raise uncertainties regarding the predictability of outcomes. Therefore, this study aims to analyze the use of L-PRF in implant dentistry by identifying its main clinical indications, evaluating its effectiveness in tissue regeneration, and discussing its limitations in surgical practice. It is concluded that an in-depth understanding of the properties of L-PRF is essential to support clinical decision-making and promote less invasive reconstructive approaches.
