Objective: to describe the distribution of dengue notifications in Maranhão from 2023 to 2025 by year, sex, age group, race/skin color, and gestational trimester. Methods: descriptive retrospective epidemiological study using aggregated secondary data from Sinan/DATASUS. Absolute and relative frequencies and annual variation were calculated. Results: 22,030 notifications were analyzed; 2024 accounted for 11,354 (51.5%) and increased by 127.0% compared with 2023, whereas 2025 recorded 5,675. Women represented 52.5%, adults aged 20-39 years were the most frequent group, and 17,083 records involved people classified as brown. Among 245 notifications in pregnant women with trimester information, 44.9% occurred in the second trimester. Conclusion: the exceptional volume in 2024 and concentration among adults support surveillance, care, and health education planning. The lower count in 2025 alone does not demonstrate a sustained reduction in transmission.
