This narrative review aimed to summarize the evidence on the use of automated devices in cardiac arrest, focusing on automated external defibrillators (AED) and mechanical chest compression devices, and discussing their advantages and disadvantages. Randomized clinical trials, observational studies, systematic reviews and national and international guidelines were reviewed. Public-access defibrillation programs shortened the time to the first shock and increased survival when the arrest presented a shockable rhythm, although the benefit is smaller in residential settings. Mechanical devices deliver constant compressions and help during transport and prolonged resuscitation, but large trials have not shown superior survival compared with high-quality manual compressions, and concerns remain about interruptions during deployment, technical failures and injuries. It is concluded that automated devices are complementary tools, and their benefit depends on training, accessibility, maintenance and integration into the chain of survival.
