The integration between periodontal health and aesthetic restorative treatments is fundamental for achieving predictable and stable outcomes. Periodontal alterations, biofilm presence, inflammation, changes in gingival architecture biological limitations and periodontal phenotype characteristics may interfere with adaptation, longevity, and stability of restorations. In this context, the objective of this study was to analyse the influence of periodontal conditions on the predictability of aesthetic restorative treatments, considering the clinical limitations of this relationship and the integration between Periodontics and Restorative Dentistry. A qualitative and comparative literature review was conducted throught searches in the PubMed, Scopus, Web of Science and Scielo databases. Studies published between 2018 and 2024 were considered and selected according to previously established inclusion and exclusion criteria. The results demonstrated that periodontal control, adequat oral hygiene, proper restorative planning, finishing, polishing and respect for the supracrestal tissue atrachment are determining factors for the preservation of periodontal tissue a and aesthetic predictability. It was concluded that periodontal health is an essential element for the sucess of aesthetic restorative treatments, reinforcing the importance of a multidisciplinary approach between Periodontics and Restorative Dentistry for individualized planning and maintenance of clinical outcomes.
