Objective: To report the case of a patient with idiopathic scoliosis whose surgical intervention for deformity correction was performed in adulthood. Methodology: This is a case report of a patient with adolescent idiopathic scoliosis (AIS) that progressed into adulthood. Data collection included analysis of medical records, imaging examinations, photographic records, and interviews. Additionally, a literature review was conducted to support the discussion of the condition, its repercussions, and the available treatment options. Case Report: A 20-year-old female patient with a history of scoliosis diagnosed at 12 years of age sought treatment due to progression of the curve to 63° in adulthood, following unsuccessful conservative treatment. She underwent posterior spinal fusion from T3 to L1, with an episode of intraoperative hypotension that was successfully reversed. The postoperative course was favorable, and the patient experienced significant improvement in her condition. Conclusion: This case supports the surgical correction of idiopathic scoliosis in adult patients, reinforcing the importance of a multidisciplinary approach and demonstrating that substantial improvement can be achieved even in cases of delayed treatment.
