This article aimed to evaluate the association between endometriosis and obstetric outcomes in pregnant women, comparing women diagnosed with the disease with those without endometriosis. This is a systematic literature review based on the recommendations of the PRISMA 2020 protocol and structured according to the PICO strategy. The bibliographic search was conducted in the PubMed database, considering studies published between 2016 and 2026, in English, Portuguese, and Spanish, involving adult pregnant women diagnosed with endometriosis and at least one obstetric outcome. After the identification, screening, and eligibility assessment stages, 15 observational studies were included in the review. Overall, the results demonstrated an association between endometriosis and an increased risk of obstetric complications, particularly placenta previa, preterm birth, cesarean section, and postpartum hemorrhage. Associations with preeclampsia, fetal growth restriction, low birth weight, spontaneous abortion, and other neonatal outcomes were also observed, although with heterogeneous results. It is concluded that endometriosis may represent an important gestational risk factor, reinforcing the need for individualized and multidisciplinary prenatal care.
