Introduction: Syphilis during pregnancy remains an important public health issue due to the risk of vertical transmission and the maternal and fetal consequences associated with delayed diagnosis and inadequate treatment. Objective: To analyze the epidemiological profile of syphilis among pregnant adolescents in the Western Macroregion of Paraná and compare it with the rest of the state between 2014 and 2024. Methods: An observational, ecological, descriptive and analytical epidemiological study was conducted using secondary data from SINAN/DATASUS. Pregnant adolescents aged 10 to 19 years reported between 2014 and 2024 were included. Results: A total of 6,709 cases of syphilis among pregnant adolescents were identified in Paraná, of which 1,680 (25.0%) occurred in the Western Macroregion. The Eastern Macroregion accounted for the highest proportion of notifications (50.1%). Conclusion: Syphilis among pregnant adolescents remains a significant public health challenge in Paraná, requiring strengthened epidemiological surveillance, improved prenatal care, and expanded preventive strategies targeting adolescent populations.
