To analyze the profile and trends of hospital morbimortality in patients aged 80+ in Paraná, Brazil (2014-2024). This retrospective study used DATASUS (SIH/SUS) data, analyzing 614,989 hospitalizations and 99,286 deaths by ICD-10 and year. The overall mortality rate was 16.14%. The main causes of death were infectious (36.98% fatality) and respiratory diseases, as well as femur fractures and intracranial trauma caused by falls, in the category of injuries resulting from external causes. The mortality rate peaked at 21.22% in 2021 during the pandemic. Length of stay correlated with mortality risk and indicated social hospitalizations (Mental disorders, 10.3 days). In conclusion, high mortality reflects structural failures in prevention (falls), hospital iatrogenesis, and care transitions, indicating the need to strengthen Primary Health Care and Palliative Care.
