Objective: to analyze the distribution of hospitalizations and in-hospital case fatality due to multiple sclerosis in Brazil by sex and age group from 2015 to 2024. Methods: ecological, descriptive, retrospective study using data from the Brazilian Unified Health System Hospital Information System. Case fatality was calculated as deaths divided by hospitalizations and multiplied by 100. Results: 49,956 hospitalizations and 317 deaths were recorded. Women accounted for 70.07% of hospitalizations, whereas case fatality was higher among men (0.98% versus 0.49%). Among people aged 20 years or older, 47,163 hospitalizations and 316 deaths occurred. Case fatality increased from 0.09% at 20-29 years to 10.47% at 70-79 years. Conclusion: hospitalizations predominated among women and younger adults, while the risk of in-hospital death was higher among men and older adults, indicating the need for differentiated care.
