Bisphosphonates are widely used in the treatment of osteometabolic disorders and cancer-related conditions; however, their prolonged use may be associated with the development of oral complications, particularly medication-related osteonecrosis of the jaw (MRONJ). This study aimed to analyze the scientific evidence regarding the impact of these complications on patients' quality of life. An integrative literature review was conducted using the PubMed/MEDLINE and Cochrane Library databases, including studies published between 2016 and 2025. Original articles, observational studies, prospective studies, systematic reviews, and reference documents addressing the effects of oral complications associated with bisphosphonate use on quality of life were included. The findings indicated that MRONJ is the primary oral complication associated with these medications and is related to significant physical, functional, psychological, and social impairments. The most frequently reported impacts included persistent pain, chewing difficulties, dietary limitations, impaired oral communication, emotional distress, and reduced social participation. Furthermore, recent evidence suggests that appropriate therapeutic interventions can significantly improve clinical symptoms and quality-of-life outcomes. In conclusion, oral complications associated with bisphosphonate use exert a multidimensional impact on patients' lives, highlighting the importance of prevention, early diagnosis, and multidisciplinary follow-up to promote better clinical outcomes and improved quality of life.
