This article aimed to analyze the therapeutic itineraries of users experiencing severe psychological distress who were transferred from a Psychosocial Care Center II (CAPS II) to a Mental Health Outpatient Clinic in a municipality in the Recôncavo region of Bahia, between 2022 and 2024. This is a documentary study based on the analysis of 127 medical records, seeking to investigate the reasons for transfers, the types of care provided, and the clinical outcomes following referral. The results showed that most users received care centered on medicalization, with 60.7% receiving only in-person medication, 26.2% receiving home-based medication administration, and only 13.1% having simultaneous access to psychotherapy. The main reasons for transfer were non- adherence to CAPS and mobility difficulties, along with a high number of cases without recorded justification. It was also observed that a significant portion of users experienced crises after referral.
